Diocese Sports Parent Consent Forms / Waivers:

All diocese sports waivers are now signed electronically at the Diocese Sports Portal. Each time you register a player for a diocesan sport in Team Snap, you will also need to log into the Diocese Sports Portal and sign the following forms electronically. This needs to be done for each participant for each sport.
- Ohio Department of Health Concussion Information Sheet
- State of Ohio Lindsay's Law ("Sudden Cardiac Arrest") Information Sheet
- Parent Code of Conduct
- Athlete Code of Conduct
All non-diocese sports waivers will be signed electronically via Team Snap during the registration process
